True Tears is loosely based upon the visual novel of the same name. The number of girls has been reduced to three; Yuasa Hiromi, the once energetic girl who lost her father; Isurugi Noe, a very peculiar girl who does strange things; and Ando Aiko, Nobuse Miyakichi's girlfriend.

Nakagami Shin'ichiro, an aspiring artist and school student, is the main protagonist of the story. Born into a wealthy household he carries many responsibilities. True Tears tells the story about his interactions with the three girls.
